Click the PhilHealth icon

Choose a HUG and add the provider

Find and choose MIAGAO RURAL HEALTH UNIT

Make sure the details are correct:

• Probinsya: Iloilo

• Banwa: MIAGAO

• Provider: MIAGAO RURAL HEALTH UNIT (Primary Care Facility)
